How This Fits Code 331

Code 331, Community Integration Supports, funds services that build the skills a participant needs for community life. It is delivered outside the participant's home. The Active Community Skills program supports these Code 331 service areas:

  • Visual, auditory, and tactile awareness and perception experiences: balance and control, rhythm and coordination.
  • Art and recreation programs: one-to-one movement coaching in gymnastics, parkour, fitness, yoga, and dance.
  • Communication skills: following cues, waiting a turn, and making choices.
  • Self-advocacy skills: participants make choices and practice asking for what they need.
  • Community awareness: safety awareness in shared spaces with other people.

Each participant's goals are drawn from their person-centered plan and IPP.

  3. 3.Connect us with the participant's FMS. We complete the FMS provider setup.
  4. 4.The family signs a service agreement with Active Community Skills covering the service, rate, and schedule.
  5. 5.Once the FMS confirms in writing, we schedule the planning session and the program begins.
  6. 6.The full program is invoiced to the FMS at enrollment. If the participant cannot finish the program, sessions not completed are refunded to the FMS.

Program Details

The program begins with a planning session. The participant, the family, and the facilitator use the person-centered plan and IPP to agree on goals for the twelve weeks. Sessions are built from five steps, combined and adjusted for each participant: Settle, Balance and Control, Rhythm and Coordination, Remember and Choose, and Finish and Review. Skills are recorded at the start, midway, and at the end, with a written summary at each checkpoint.
